What Is Medicare Part D?

Medicare Part D is the prescription drug benefit added to Medicare in 2006. It's provided through private insurance companies approved by Medicare and is available as either a standalone Prescription Drug Plan (PDP) — added onto Original Medicare — or bundled into a Medicare Advantage plan (MAPD).

If you have Original Medicare with a Medigap plan, you'll need a standalone Part D plan for drug coverage. If you have Medicare Advantage, drug coverage is usually already included.

Late enrollment penalty: If you go without creditable drug coverage for 63 or more consecutive days after becoming eligible, you may owe a permanent late enrollment penalty added to your monthly premium. Don't wait — we'll make sure you're enrolled on time.

How Part D Pricing Works

Understanding the structure helps you see why comparing plans — not just picking the cheapest premium — matters.

Formularies Vary by Plan

Each Part D plan has its own drug list (formulary) with different coverage tiers. A drug covered at Tier 1 on one plan may be Tier 3 on another — meaning dramatically different costs at the pharmacy.

Premiums + Cost-Sharing

Part D plans charge a monthly premium plus copays or coinsurance at the pharmacy. The lowest-premium plan is rarely the lowest total cost — that depends on your specific medications.

The Coverage Gap (Donut Hole)

After you and your plan spend a certain amount on drugs, you enter a coverage gap phase. Costs during this phase depend on your plan and whether your drugs are brand or generic.

Catastrophic Coverage

Once your out-of-pocket drug costs reach the catastrophic threshold for the year, your copays drop significantly for the rest of the calendar year.
